The form contains input fields for just the most important details, such as name, email and a place to add some comments. There is a PHP back-end script that makes the form work. On this page the displayed form lacks the PHP back-end script, however, because it cannot be used on the CC S-drive.
Before you can use the form, you need to open this script and edit three lines:
On line 14 you see the mail address 'siteowner@domain.com'. It has to be changed to the real siteowner's mail address, and preferrably an address pertaining to the domain where the form is located.
Then on line 33 there is the mail address again. Most servers prefer it to be the same address as on line 14. If the server allows it, you may use a different address for both these lines.
Finally, on line 67 you can edit the URL to a page or site that you want to redirect the visitors to after the form submission.
