This form has to be filled in online, then downloaded and saved as a .pdf. It can be submitted by email or 'snail mail'. An empty form can also be printed and handed out to people who want to fill it in by hand.

You can add 'input fields' (in this case empty paragraphs) for details you need to ask about, like phone number, street address, website, various options to select from or check. For restaurants you can add the whole range of menus and let your guests select what they want from home, before they even come to the place. You have to add the attribute contentEditable = "true" to the new input fields to make them work.
